Job Description

Are you a compassionate and experienced LPN who loves working with children? Join Applied Home Health Network, a trusted name in pediatric home health care. We are urgently hiring Pediatric Licensed Practical Nurses (LPNs) who are ready to make a lasting difference in the lives of medically fragile children in a home-based setting.

What You'll Do:

• Conduct comprehensive assessments and monitor the health of pediatric patients.
Administer medications with pediatric-specific dosing based on age, weight, and surface area.

Provide hands-on care for children with G-tubes, tracts, ostomies, and ventilator needs.

Collaborate with physicians, case managers, and therapists to carry out individualized care plans.

Educate and support families in managing pediatric conditions at home.

Maintain detailed and accurate documentation using Axxess or other EHR systems.

Perform pediatric medical-surgical procedures while ensuring a child-friendly, trauma-informed approach.

Assist in managing pain and providing dialysis support when needed.

Who We’re Looking For:

• Licensed Practical Nurse (LPN) in good standing with the State of Illinois.
• CPR certification required; PALS certification is a strong plus.
• 3+ years of pediatric experience in home health, medical-surgical, or acute care.
Proficient in airway management, tracheostomy care, and infant support.

Experience with long-term care, medically complex children, or developmental disorders is highly valued.

Excellent communication skills and the ability to work with families and care teams.

Tech-savvy with experience using electronic health records (EHRs).
