Overview
Applied Home Health Network is urgently seeking a compassionate and skilled Pediatric Registered Nurse (RN) to join our growing team. If you’re passionate about providing exceptional care to children in a supportive home environment, we want to hear from you today! In this role, you will provide high-quality care to pediatric patients in a home health setting. The ideal candidate will possess strong clinical skills, including proficiency in airway management, infant care, and pain management. Position Overview:
The Home Care Supervisor will oversee all aspects of the Community Care Program (CCP) including client referrals, staff training, documentation compliance, and supervisory duties. The ideal candidate is detail-oriented, proactive, and experienced in Illinois Department on Aging (IDoA) programs and compliance.
Key Responsibilities
Administrative Duties:
- Answer and route client and caregiver calls
- Provide exceptional customer service and timely responses to inquiries
- Maintain organized and confidential client and employee filing systems
- Lead the establishment of AHHN’s HCA
- Build and maintain relationships with CCUs (Case Coordination Units) and community referral sources
- Manage intake and referral processing for new CCP clients
Supervisory Responsibilities:
- Onboard, and supervise HCA staff
- Conduct and document pre-service, quarterly, and in-service training
- Coordinate care plans and caseload management
- Conduct quarterly performance evaluations and file reviews
- Ensure proper and timely documentation of services and incident reports
Compliance & Documentation:
- Maintain accurate electronic records of employee/client data (AXXESS)
- Manage client assessments and visit documentation per IDoA guidelines
- Conduct audits for service deviations or noncompliance
- Ensure adherence to all CCP policies and procedures
- Process timesheets and manage payroll reports
- Submit accurate payroll data
Client & Community Engagement
- Refer clients to appropriate social service programs as needed
- Assist clients and families with program applications and eligibility requirements
- Ensure client satisfaction and address concerns or complaints within 24 hours
- Promote independent living and dignity for clients through quality home care services
Required Qualifications:
- Strong verbal and written communication skills in English (Spanish or other languages a plus)
- Proven ability to manage time, files, and shifting priorities effectively
- Intermediate to advanced computer skills (Microsoft Office, cloud-based platforms)
- Minimum of 2 year of office or supervisory experience in home care or healthcare
- Willingness to complete the Home Care Supervisor Certification within 3 months of hire
Preferred Qualifications:
- Familiarity with Axxess or similar home care documentation platforms
- Working knowledge of IDoA and CCP regulations
- Prior experience supervising or coordinating home care aides
- Experience scaling or managing CCP caseloads

Pay: $19.50 - $22.00 per Hour
